2014, KneeCitation Excerpt :Osteoarthritis is the single most common cause of disability in older adults, with approximately 10% of patients over 55 years of age affected by painful knee osteoarthritis causing mild to moderate disability [1]. Intra-articular corticosteroid injections are often administered for painful arthritic conditions including osteoarthritis and rheumatoid arthritis, an accepted treatment for many decades [2–7]. A survey from the USA has shown that more than 95% of rheumatologists administer intra-articular steroids [8] and injections are also commonly administered by other health care professionals including Orthopaedic surgeons and General Practitioners (GP) [9].

Because nonsteroidal anti-inflammatory drugs and analgesics would provide significantly greater pain relief than placebo [31], continuous use of these mediations during the experimental period may mask the efficacy of GlcN. Two of the studies reviewed did not encourage the use of rescue medication [10,32] and hence less than 9% of participants in these studies had consumed rescue medication (Table 2).

Also, there is theoretical possibility that intra-articular saline could have a therapeutic effect to that may be sustained for 6 months or more73–75. Furthermore, a therapeutic effect of aspiration of synovial fluid could contribute to a response in patients receiving placebo.
