HLA-alloantigen associations in Greek patients with Sjögren's syndrome

Abstract

The frequency of HLA-A, -B, and -DR alloantigens was studied in 46 patients with primary Sjögren's syndrome (pSS), 14 patients with secondary Sjögren's syndrome (sSS) and rheumatoid arthritis (RA), 26 classical RA patients without clinical or histologic evidence of Sjögren's syndrome (SS) and 172 normal controls.

A statistically significant increase in the frequency of HLA-DR5 alloantigen was observed in the pSS patients, compared with the controls (P < 0.007, corrected). No differences were detected between pSS patients with or without extraglandular manifestations, nor were any found between anti-Ro(SSA) positive and negative patients.

The frequency of HLA-DR3 alloantigen (30%) was not significantly higher in our pSS population than in controls (25%), nor was that of -DR4 in the RA or the sSS patients.

The results of the present study indicate that the ‘SS susceptibility gene’ in Greek patients is linked with the HLA-DR5 expression gene and they support the hypothesis that the fundamental pSS gene may belong to another locus.

      The authors confirmed HLA-DQA1 ∗ 001-DQA1 ∗ 0201-DQB1 ∗ 0501 in Israelite Jews and in Greeks HLA-DQA1 ∗ 050 [26]. Five years prior, HLA-DR5 was found to be one of the susceptibility genes in SS Greek patients [27], and in 2004 an extensive study examining Sjögren's syndrome associated with Systemic Lupus Erythematosus revealed that in these patients a high frequency of the HLA-DRB1*0301 allele was present [28]. Significant associations were also found in primary SS patients from southern Spain with HLA-Cw7, HLA-DR3 and HLA-DR11 [29].

      The initial search strategy identified 73 studies including review articles (Fig. 1 and Supplementary Table 3) Through the screening of the abstracts and titles, 52 association studies relating to HLA-DRB1, HLA-DQA1 and/or HLA-DQB1 polymorphisms and susceptibility to pSS were left for data extraction. Twenty eight articles were excluded (Supplementary Table 4) and a total of 23 articles for HLA-Class II region were included in the final meta-analysis [9–16,18–19,27–39]. A total of 1166 cases and 6470 controls were analyzed.
