Sialochemistry for diagnosis of Sjögren's syndrome in xerostomic patients

Abstract

The flow rate and composition of whole unstimulated saliva were measured in fifteen healthy controls and in forty-eight xerostomic patients, fourteen suffering from xerostomia per se, twenty-two from xerostomia with keratoconjunctivitis sicca (KCS), and twelve from xerostomia, KCS, and rheumatoid arthritis. A significantly lower salivary flow rate was found in all the xerostomic patients. Sodium, potassium, and IgA concentrations were significantly elevated in the KCS and in the RA + KCS group in comparison with the patients who had xerostomia per se and with the healthy controls. Sialochemistry is thus recommended for the diagnosis of Sjögren's syndrome in xerostomic patients.

The prevalence of xerostomia was considerable lower (6%) in a study of Andonopoulos et al.,33 but it was higher (26.25%) in a study by Jensen et al.8 Such a different results may be partly explained by different approaches in the assessment of xerostomia as well as by differences in the patients. Xerostomia in our RA patients was accompanied by a significant reduction of salivary flow compared with the control groups (RA patients without xerostomia and the healthy control group); however, it appeared higher at an unstimulated salivary output than usually accepted in the literature.34-36 The significant reduction in the salivary output in RA xerostomic patients may indicate dysfunction of the secretory cells of the salivary glands as one of the major targets of xerostomic RA.
The aim of this study was to assess the relationship between complaints of xerostomia in patients with rheumatoid arthritis (RA) with the total output of the salivary proteins of innate and adaptive immunity.
The salivary output and specific activity of peroxidase and specific contents of lysozyme, lactoferrin, and secretory immunoglobulin A (sIgA) were determined in xerostomic RA patients, nonxerostomic RA patients, and healthy control subjects.
Compared with nonxerostomic RA and healthy control groups, xerostomic RA patients had significantly decreased output of saliva and protein, decreased peroxidase activity, and a significantly lower specific content of peroxidase and sIgA. Compared with the RA control group, xerostomic RA patients had significantly lower specific content of all salivary proteins examined.
The results indicate that xerostomia in patients with RA may be a harbinger of diminished saliva production regarding quantity and quality, and may be indicative of impairment of the salivary immune system of the oral cavity in xerostomic RA patients.

In the current report, the main proteomics techniques and their application to human disease study are reviewed.
Saliva: An additional diagnostic tool in Sjogren's syndrome

To assess the available data and the place of salivary analysis in thediagnosis of Sjögren's syndrome (SS).
A Medline search of English language articles published between 1985 and 1996 and a manual search of the reference lists of relevant articles formed the data sources. These were combined with our clinical and experimental experience in this field. Each method of salivary analysis was assessed according to study design, type of saliva used for the study, sensitivity/specificity for the diagnosis of SS, and correlation to the histopathological findings.
Increased levels of salivary Na⁺, immunoglobulins (particularly IgA), anti-Ro and La antibodies, lactoferrin, lysozyme, β₂ microglobulin, prostaglandin E₂, thromboxane B₂, interleukin-6, and hyaluronic acid have been detected in various studies. Results varied according to the different methods used for saliva collection.
Although many changes have been detected in various constituents of saliva in SS patients, no test has proved sensitive or specific enough for diagnosing SS.

The salivary composition and flow rate were examined in 20 patients with insulin-dependent diabetes mellitus (IDDM) and in 19 patients with non-insulin-dependent diabetes mellitus (NIDDM) and compared with 20 healthy controls. Resting and stimulated whole and submandibular saliva was analyzed. Significantly lower resting salivary flow rates were found in the IDDM patients as compared to the NIDDM group. In the IDDM patients potassium concentration in resting saliva was significantly higher compared with healthy controls and in stimulated whole saliva compared with NIDDM patients. No difference in salivary total protein, amylase, lactoferrin, or lysozyme was found among the three groups examined. The IgA concentration of the IDDM patients was significantly higher in whole resting saliva compared with controls and in the submandibular saliva compared with both NIDDM patients and controls. No difference was found between controls and the diabetic patients examined in prevalence of complaint of dry mouth. The salivary flow rates, however, were significantly lower in the three subgroups with dry mouth compared with the subgroups without this complaint. Caries were detected in 100% of the diabetic patients and controls. No correlation was observed between the incidence of caries and any of the salivary parameters examined. A higher prevalence and severity of periodontal disease was detected in the diabetic patients as compared to the controls. A significant positive correlation was found between the gingival index and the concentrations of total protein, albumin, lysozyme, and lactoferrin in whole resting saliva in the three groups examined.
Periductal lymphocytic infiltration of salivary glands in Sjögren's syndrome with relation to clinical and immunologic findings

We studied 113 patients who were suspected to have Sjo¨gren's syndrome (SS) because they had dry mouth and dry eyes, and who were determined as not having any other autoimmune disease, to clarify the relationship of periductal lymphocytic infiltration of salivary glands to clinical and immunologic findings in relation to SS. Periductal lymphocytic infiltrations were observed in the labial and/or the parotid glands of 57 patients (P-group). The salivary glands of the other 56 patients did not demonstrate obvious histologic changes (N-group). The stimulated salivary flow rate of parotid glands of the patients in both the P- and N-groups was significantly reduced when compared with healthy persons. However, no difference in the flow rate was observed between the P- and N-groups. In contrast, the percentage of patients in the P-group with keratoconjunctivitis sicca was significantly higher than that of patients in the N-group. Percentage of γ-globulin fraction and IgG level in the sera of the patients in the P-group were both significantly higher than those in the N-group. The percentages of patients who demonstrated the serum rheumatoid factor, anti-SS-A, and anti-SS-B antibodies were also significantly higher in the P-group than in the N-group.
