Abstract

Elastofibroma dorsi was diagnosed in a 76-year-old woman with bilateral subscapular tumor masses and a 3-year history of recurrent, right-sided scapular pain. A bilateral tumor extirpation was performed and the patient left the hospital free of symptoms 10 days later. We suggest that both constant trauma with resulting excessive elastin production and collagen degeneration might play a major role in the pathogenesis of this rare proliferative lesion.

Dorsi elastofibroma. About 14 cases, and review of the literature

L’élastofibrome dorsal est une tumeur bénigne siégeant le plus souvent au niveau des tissus mous périscapulaires de la paroi thoracique postérieure. Nous rapportons une série rétrospective de 14 cas d’élastofibromes dorsaux, chez neuf patients, pris en charge en chirurgie plastique, viscérale et orthopédique. L’âge moyen était de 67 ans. Cinq patients présentaient des lésions bilatérales. Toutes les lésions siégeaient dans la région sous-scapulaire. Neuf lésions ont été retirées chirurgicalement et six épaules sont asymptomatiques après la chirurgie. L’élastofibrome dorsal est une lésion rare, d’évolution lente. On le trouve le plus souvent chez les patients d’âge mûr. L’étiopathogénie de cette lésion est pour l’instant mal définie. L’imagerie par scanner ou IRM permet de poser le diagnostic d’élastofibrome dorsal, et peut permettre, dans les cas asymptomatiques et typiques, d’éviter la biopsie et la chirurgie. Seuls les cas symptomatiques doivent être opérés.
Elastofibroma dorsi is a benign tumor which usually occurs at the inferior angle of the scapula. We retrospectively reviewed 14 cases of elastofibroma dorsi, in nine patients. The patients were hospitalized in the departments of plastic surgery, orthopedic surgery or visceral surgery. Mean age was 67 years. The location of the lesions (bilateral in five patients) was typical, in the thoracoscapular region. Nine lesions were removed surgically, six shoulders were asymptomatic after surgery. Elastofibroma is a rare, slow-growing lesion. It occurs in connective tissue of the infrascapular region of elderly patients. The pathogenesis of the lesion still remains unclear. MRI and computed tomography are useful for assessment of elastofibroma dorsi, and can potentially help avoid the need for unnecessary biopsy and surgery, especially in the asymptomatic patient. It should be surgically removed only in symptomatic patients.

L’élastofibrome est une pseudotumeur rare, naissant habituellement à partir des tissus mous entre la pointe de la scapula et la cage thoracique. Elle touche essentiellement les femmes âgées et les travailleurs manuels. Sa présentation clinique et ses aspects radiologiques permettent souvent de poser le diagnostic. Il est parfois difficile de ne pas envisager la biopsie afin d’éliminer un sarcome. Nous rapportons trois cas d’élastofibrome sous-scapulaire.

Elastofibromas are rare benign soft tissue tumours that are usually located between the scapula and the rib cage deep in the serratus anterior muscle. Their anatomical location, distinctive clinical symptoms and radiological characteristics set them apart from malignant soft tissue tumours. Although they are rare, it is necessary to be aware of this benign tumour to avoid unnecessary biopsies; surgical resection may, however, be recommended to obtain a differential diagnosis from malignant sarcomas. We report three cases of elastofibroma dorsi in a 48-year-old man, a 33-year-old woman and a 42-year-old man.

L’élastofibrome (EF) dorsal est une tumeur bénigne siégeant essentiellement au niveau de la paroi thoracique postérieure, habituellement au niveau des tissus mous périscapulaires. À travers notre observation et une revue de la littérature, nous tenterons de préciser les caractéristiques de cette tumeur et les modalités de prise en charge. Nous pensons que cette entité gagnerait à être connue de tout praticien, ce qui permettrait une attitude thérapeutique codifiée et adaptée.
Elastofibroma dorsi is a slowly-growing benign tumor which usually occurs at the inferior angle of the scapula. It may be more of a degenerative process than a neoplasia. We seek, through our observations and a review of the literature, to clarify the characteristics of this tumor and its modes of management. We think that all doctors should be aware of this condition, in order to treat it correctly.

Estudio y comparación del elastofibroma dorsi y distintas tumoraciones de la región escapular.
Se ha realizado un estudio retrospectivo de nueve tumoraciones de partes blandas que afectaron a la región escapular, analizando los métodos de diagnóstico y sus diferentes tratamientos.
De las nueve tumoraciones estudiadas, cinco correspondieron a elastofibroma dorsi, y las cuatro restantes a tumoraciones de diferente estirpe, dos liposarcomas mixoides, un lipoma intramuscular, y una ruptura muscular y hematoma secundario. Aun siendo los elastofibromas raros en la práctica clínica diaria, el conocimiento de las características de esta lesión debería disminuir los diagnósticos erróneos y hacer innecesarias cirugías agresivas y, probablemente, biopsias.
Study and comparison of elastofibroma dorsi and other tumors of the scapular region.
Nine soft-tissue tumors of the scapular region were studied retrospectively to analyze diagnostic methods and treatments.
Of the nine tumors studied, five were elastofibromata dorsi and the remaining four included two myxoid liposarcomas, one intramuscular lipoma, and a muscle tear with secondary hematoma. Although elastofibromas are rare in daily clinical practice, familiarity with the characteristics of this lesion would reduce erroneous diagnoses and reduce the need for biopsy and aggressive surgery.
