SP0150 The Care of Teenagers with JIA - Efforts Done to Improve the Transition Process in Finland

During the last years many efforts have been done to improve the transition process in Finland. The routines differ throughout the country and these different routines are presented. In many centres we start preparing the youngsters at the age of 12-13 years. We invite them into the examination room without parents. After the examination the parents are welcomed into the room for discussion. Transition readiness checklists are used. Special transition days are arranged. At some centres a Web-based evaluation of the guidance given at the paediatric clinic is performed at the first visit at the adult clinic. The goal is to prepare the teenagers gradually and according to their psychological development.
