Abstract

Background Osteosclerosis is widely known as a radiological sign of osteoarthritis. Nowadays the classical film radiography is more often replaced by the digital medical imaging.

Objectives To find out if the osteosclerosis is present within hand and knee from the doctor's point of view and by using a program of digital medical imaging along with classic X-ray and to establish a correlation between results.

Methods 103 patients were assessed by doctor and special designed software. As for the program the medical X-ray was segmented and subsequently the bone sections histogram along with several metadata of the image were analyzed by a fuzzy inference system. 206 bone sections called anatomical areas were assessed by the program. The metadata contains the technical characteristics of the image.

Results Following the assessment there were discrepancies only in 24 anatomical areas (11.65%), in 15 (7.28%) of them the osteosclerosis was emphasized by the program. As it is pointed out below, the osteosclerosis was found especially within the 1st phalanx of finger 1 along with distal extremity of ulna, 2 areas involved in osteoarthritis.

There were standing out linear correlations between the assessment carried out by doctor and the assessment achieved by using the program regarding the whole hand (0.949) and the whole knee (0.924).

Conclusions By using a special program the osteosclerosis could be emphasized as of the first symptoms of osteoarthritis, therefore the patient undergoes a proper treatment with regard to the patient and its quality of life along with positive impact on medical system. Moreover the use of further investigation based on X-rays is prevented. Owing to this fact this program could turn out to be useful in order to help the doctor's assessment.
