We focus with significant interest on the report by Lv et al,1 which was related to comparison of Tripterygium wilfordii Hook F (TwHF) with methotrexate (MTX) in the treatment of active rheumatoid arthritis. Lv et al found that TwHF monotherapy was not inferior to, and MTX+TwHF was better than, MTX monotherapy in patients with active RA.

However, we think that there are still several critical questions in this research, and one of them should be treated carefully in revealing the results of this trial.
